Having done many handy man repairs as part of my business in homes I'd say your flapper in side the tank needs to be raplaced. Simple job and local hardware store should have the part along with easy instructions to replace - Thanks - Harry Conant - harrys real wood furniture

Lift up the back of the toilet(cover on top of tank). Underneath there, you should see some kind of float, and a shuttle system that pulls up the plug on the bottom of the tank. This is clean water, so don't have a hissy. Just push the float down under the water, and it will flush. Or, grab the shuttle(this involves reaching in the water) and pull up.

Try to shorten the chain that goes to the rubber flapper and if the doesn't work to resolve the problem the flapper may need to be replaced, They do wear out over time.

This is uaually that the chain that attaches to the rubber flapper in the bottom of the tank needs to be adjusted shorter or that the flapper could use to be replaced. they do wear out over time.
