    Raleigh, NC - "Liposuction in a doctor's office?

    Q: My doctor’s office is medi-care certified and he said he could perform my liposuction there, instead of in a hospital. Is this safe?

    A: Liposuction, as you may have heard in the popular media, is the most commonly performed surgical procedure each year in the United States. Here's a statistic that you may not have heard: the majority of physicians performing liposuction in the United States are not plastic surgeons; in fact, many do not have any formal surgical training whatsoever. It seems hard to believe, but many physicians performing liposuction have had no more training in liposuction than a 'weekend course'. One way to determine whether or not a physician has had appropriate training in a particular surgery is to confirm that they have hospital privileges for that procedure.

    I fear that some practitioners view liposuction as a 'simple' surgery, since it does not involve making large incisions, and it requires little, if any, suturing. Nothing could be further from the truth. Liposuction, in my mind, is a very challenging operation that requires careful planning and preparation, and a great deal of care and finesse when it is actually performed. It requires a three-dimensional understanding of the layers of human anatomy, an understanding that is second nature to a surgeon alone. I think that it is often an inadequate understanding of anatomy (and, perhaps, of the body's response to surgery) which leads to the poor results in liposuction and body contouring that unfortunately are so often seen.

    It is important that your physician has hospital priveleges for the procedure you are interested in having performed.  A hospital has access to information and records that the general public does not. Hospitals also have tremendous exposure to liability for cosmetic surgery procedures performed within them. They therefore will only grant surgical privileges to physicians that can demonstrate appropriate training and experience.

    Although many aesthetic cosmetic surgery procedures are not performed in hospital operating rooms, the fact that a hospital has granted a surgeon privileges for a given procedure ensures that the surgeon has met an accepted standard of competence. It also means that your surgeon will be able to take care of you at a hospital should any complications from cosmetic surgery arise.

    If you are planning to have cosmetic surgery performed, you should do your due diligence to determine if your surgeon has the appropriate credentials. This issue can be settled by the following simple question: Does my surgeon have hospital privileges to perform my cosmetic surgery?

    What are the Benefits of Botox? Raleigh, NC

    My Raleigh, North Ccarolina Plastic Surgery Practice is one of the top users of Botox in the country acccording to our Allergan representative. it is one of the most popular procedures we offer.

    BOTOX ® is a protein complex that interferes with muscle contraction. It temporarily blocks nerve stimulation of muscles where it is injected, so that muscle contraction is weakened or inhibited. BOTOX ® has been safely used for many years to treat spasms of the muscles that move the eye (strabismus) and uncontrollable blinking (blepharospasm). In 2002 it was approved by the FDA for cosmetic use, and since that time it has been marketed as BOTOX ® Cosmetic.

    Many facial wrinkles and creases are active lines caused by muscle contraction, which can be blocked by BOTOX ® injection. The effect is temporary, usually lasting 3 to 6 months. With repeated injection, the effect may regularly last up to 5 or 6 months. Although some wrinkles and creases may completely disappear (temporarily), it is important to understand that some may improve but not completely disappear. Some people (very few) are resistant to BOTOX ® and achieve little or no response.

    Where does BOTOX ® work the best?

    Areas of the face that respond well include 'crows feet' lines around the eyes, vertical frown lines between the eyebrows, and horizontal ('worry') lines on the forehead. BOTOX ® can also be administered just below the lateral aspect of the eyebrow to produce a temporary lateral browlift, which opens up and brightens the eyes.

    In some instances, BOTOX ® can be used to soften vertical upper and lower lip lines, to reduce muscle bands in the anterior neck, and even to elevate the nasal tip. While there are some good 'lower face' indications for BOTOX ® administration, in my practice the majority of injections are for 'upper face' (especially eye area) rejuvenation.

    One truly remarkable consequence of BOTOX ® administration that I have seen over and over again is the gradual complete disappearance of deep, vertical furrows between the eyebrows (frown or 'scowl' lines). With long-standing muscle contraction, the vertical lines between the brows may become actual physical clefts or grooves in the dermis, resulting in creases that are still apparent even when the skin is spread apart tightly with a finger on each side of the crease. In such instances, the initial BOTOX ® injection will soften but not completely eliminate the deep furrow. With regular BOTOX ® treatment, however, such that the frown muscles are never allowed to fully regain their normal ability to contract, the deep furrows may gradually fade completely.

    Some patients with chronic, deep furrows between the brows achieve improvement but not complete correction of these creases with repeated BOTOX ® injection. In these cases, a soft tissue filler can be used to eliminate what remains of the furrow. I have used Restylane for this indication in a number of patients with excellent results, and for several of these patients I have performed structural fat grafting of the remaining furrow to provide a permanent correction.

    Mini Tummy Tuck Raleigh, NC

    The nature of the tummy tuck surgery that will best suit each patient is determined during the consultation and is based on the physical examination. For all patients, I draw the proposed incisions on their body with a skin marker (it washes off easily) so that it is clear what the nature of the resulting surgical scars will be. I also outline liposuction areas if that is part of the surgical plan.
    In some people, the abdominal skin will contract after delivery and maintain a great deal of its natural tone. Abdominal bulging may be confined to the area below the belly button. In these patients, a mini-tummy tuck may be adequate, which involves a shorter suprapubic incision, no surgical alteration of the belly button, and a quicker recovery time.

    In a mini-abdominoplasty, the rectus abdominis muscles may be tightened below the belly button only, if the abdominal "bulge" is confined to that area. If abdominal wall laxity extends above the belly button, it is possible to tighten the muscles all the way up to the level of the sternum (ribcage). To do this, the umbilical stalk is divided at its base, and then reinserted at its original location once the muscle-tightening sutures have been placed. The patient still keeps their 'original' belly button.

    A mini-tummy tuck generally does not alter the blood supply of the abdominal skin as much as a full tummy tuck, and therefore more aggressive liposuction of the waist and back may be performed in many cases.

    Raleigh, NC "What's the best way to treat sunken cheeks?"

    In my Raleigh, North Carolina Plastic Surgery Practice I use Fat Grafting to treat sunken cheeks.  There are  a number of 'soft tissue fillers' are available for temporarily improving facial areas that have lost volume or have developed deep lines and creases. Probably the most popular fillers currently are hyaluronic acid products like Restylane, Perlane or Juvederm which can be used to plump up thin lips and to fill out nasolabial folds (lines that run from beside the nostrils to the area beside the corners of the mouth) and marionette lines (lines that run from the corners of the mouth towards the jawline). The improvement generally lasts about 6  months and can last up to a year. I regularly use Juvederm, Restylane and Perlane in my Raleigh, North Carolina plastic surgery practice to fill lines and plump lips

    For patients seeking permanent enhancement of soft tissue volume, the ideal material to use would obviously be something that is naturally-occurring and not rejected by the body or treated by the immune system as a foreign material. Whether the goal is to restore volume to an area such as the lips or the cheeks, or to fill in a crease or depression such as the nasolabial folds or marionette lines, the ideal material is quite clearly the material that one wishes was there in greater abundance in the first place: YOUR OWN FAT.

    A relatively new technique has been developed called structural fat grafting, in which small amounts (less than 0.1 cc at a time) of fat are carefully microinjected in a series of discrete layers to gradually 'build' new soft tissue structure. As there is space between each microinjection, new blood vessels are able to grow into the grafted fat, allowing it to persist. If this process of blood vessel ingrowth (neovascularization) does not occur, then the injected tissue cannot truly be considered a 'graft' and is instead just another 'soft tissue filler' of limited duration.

    This is a procedure that requires specialized training and specialized surgical instruments, as well as patience and attention to detail on the part of the surgeon. When performed properly, permanent improvements in facial plastic surgery are possible.

    Raleigh, NC - "Will I Have Visible Scars from a Face Lift?"

    The facelift procedure involves incisions that skirt the contour of the ears, using the anatomy of the ear to help conceal them. For a full facelift, the incision starts in the sideburn area, follows the contours of the junction of the ear with the face, curves behind the earlobe into the recess between the posterior ear and the neck/scalp, and then extends into the hairline posteriorly at the top of the ear. When I make these incisions, I design them so that, once fully healed, they may be difficult for even a hairdresser to detect. That goal can often be achieved, and it requires meticulous attention to every centimeter of the closure.

    Which Breast Implant Placement Gives a More Natural Appearance?

    While the issue of ‘over’ or ‘under’ the pectoralis major muscle receives a great deal of attention, even more important than implant position relative to this muscle is implant position vertically and horizontally on the chest wall.  In many patients, the inframammary fold needs to be lowered in order to allow the implant to rest at a level that appears natural relative to the position of the nipple and areola, and in order to prevent the appearance of excessive upper pole fullness.  In my Raleigh North carolina Plastic Surgery Practice, I regularly place breast implants below the muscle and above the muscle.

    In profile, the natural-appearing breast is not convex in the upper pole, and an excessively convex and overly full upper pole is a dead giveaway that an implant sits below the skin. Likewise, if the inframammary fold is lowered too far, the augmented breast will appear ‘bottomed out’, with an excessively full lower pole, an empty upper pole, and a nipple/areola that appears to sit too high on the breast – another situation with a distinctly unnatural appearance.

    The horizontal position of breast implants also requires a great deal of attention, both in pre-operative planning and in the operating room. Breast implant pockets that extend too fat laterally will result in augmented breasts with an excessively wide space between them in the cleavage area, and the appearance that the breasts are abnormally far apart.  If the pockets do not extend far enough laterally, however, the result is an augmentation with an abnormal ‘side by side’ appearance. As it is the lateral projection of the breasts beyond the lateral limit of the chest wall (in frontal view) that, along with the concavity of the waist profile and the convexity of the hip profile, produces the appearance of an ‘hourglass figure’, careful attention must be paid to ensure that lateral breast projection is adequate and appropriate.

    Another consideration is that the implant base diameter must match the existing anatomic limits of the breast preoperatively and the breadth of the anterior chest in general. Obviously, a given implant volume and diameter that works well for a small-framed patient who is 5’2” will be inadequate for a large-framed patient who is 5’10”. Careful evaluation of all of these issues is necessary if the ultimate goal of the surgery is a natural-appearing breast enhancement.

    I urge all propective patients to take time, do a lot of research and view many many before and after photographs before selcting a plastic surgeon to perform your breast augmentation. Taking some taime to speak to several former patients who have had the same surgery you are considering can also be helpful.
