I've gone back to my Maiden name and need to change the name on my Title/Registration. Can you help me with this and what documents are required?

You will need to go the the Department of Motor Vehicles office to do this. You will need to also change the name on your drivers license and they recommend that you change your social security card prior to doing the change on your drivers license and auto title.
You will need to bring your social security card, original title, birth certificate, and a copy of your divorce decree.
There will be some fees charged to change the name. For more info you can contact the Louisiana Department of Motor Vehicles at 225-925-6146.

We provide the notary service required to effect the transfer. We will prepare an Act of Donation, ensure that it is correctly executed and notarized, and will ensure that the back of the current title is correctly filled in and notarized. Our fee for this service is $60 (most areas). We will come meet you and the donor of the vehicle. Once these documents are signed and notarized you will need to take them to the Department of Motor Vehicles Office to receive a new license plate and title.
