I don't think there is a specific time schedule of taking communion, some churches do it once a month others do it once every 3 months depends on the pastor.

And it should NOT be taken with real wine, for the fact that everyone in the church is overcoming something and we all have a weight that so easily besets us you don't want to be a stumbling block for others.

Grape Juice will serve the same purpose, it is symbolic of the wine.
