How to Request a Copy of a Naturalization Certificate

Foreign citizens must first qualify for, and then apply to become a naturalized citizen of the United States. When this process is completed, a naturalization certificate is issued which provides proof of U.S. citizenship. A copy of a naturalization certificate can be sought if the original becomes lost, damaged or destroyed, a name change has occurred or if the original certificate was issued incorrectly.

  1. Log on to the United States Citizenship and Immigration Services, or USCIS, website.

  2. Click on the tab, which reads Forms. Under Forms, find and click on the link titled, “Citizenship and Naturalization-Based Forms.”

  3. Scroll down to form N-565, the application form to replace a naturalization certificate. Click on the form name to open a page with information about the form.

  4. Open the link with the instructions on how to fill out the form. Read the directions thoroughly. If the instructions are not followed correctly your request for a copy of your naturalization certificate may be denied.

  5. Print form N-565 and complete the application, using black ink.

  6. Fill in all of the information thoroughly and accurately.

  7. Attach the damaged document to the application form if you are requesting a copy due to damage. Attach a copy of your marriage certificate if a naturalization certificate copy is necessary due to name change because you got married.

  8. Include two copies of a color picture of yourself taken within 30 days of filing form N-565. The pictures must be identical, have a white background and show you face forward.

  9. Print your name and registration number on the back of the picture with pencil or a felt pen.

  10. Write a check for $345 made out to “U.S. Department of Homeland Security” for the filing fee. The check must be in U.S. dollars and be from a bank in the United States.

  11. Include the filing fee, original naturalization certificate, two photos and completed form N-565 an envelope and mail to the proper location. The USCIS website will tell you the address to which your forms must be mailed, based on where you live. This information is included in the instructions on how to complete form N-565.

  12. Wait for the USCIS to contact you with either your approved naturalization certificate copy or a request for further information.
